They had their chance, in the face of a clear public mandate for reform in healthcare. It's time to take the obfuscatory "bull" by the horns of our present dilemma and call a spade a catastrophe. Any other situation rendering so many dead from a cause would put a National Emergency red light on, granting the President special powers; which in this case means him giving Medicare to all who need it, until each state takes the initiative or Congress does. The simplicity of this approach (talk about economical; they wouldn't even have to buy new stationery!) would appeal to a public dying for action, and win kudos to an administration being faulted for its lack of balls.
